Libertus is using the Outcome STAR!
At Libertus we want to ensure that our Members feel empowered in all aspects of their life and to ensure that we help our Members feel this way we have implemented the Outcomes Star as a means of helping them achieve this. We encourage our Members to use the Outcomes Star as a method for personal growth and development – it also allows us to get to know our Members better, and to better understand their needs.
The Outcome Star is a support and evaluation tool designed to help individuals identify personal goals for change within their life. The Outcome Star is used all over the world by many different services all of whom are providing a caring or supportive role for their client base. The Outcomes STAR benefits our members and staff by allowing us to update your, preferences, needs and agreed outcomes, ensuring your day centre experience and support is tailored to you and your choices.
When initially completing the Outcomes Star Members are asked to think of key areas (or goals) where they may wish to make a change: for example it may be in relation to their health, home life, hobbies etc. The Outcomes Star provides a framework for conversation between our Members and staff, and supports both parties to reflect on the strengths and challenges that exist when working towards their personally set goals.
Having completed a Star collaboratively, Members and staff can then use the stages provided by the Outcome Star to inform a shared action plan – targeting goals, actions and interventions. The Outcome Star also allows Staff at Libertus to reflect on best practice, and allows Care Practitioners to provide a fully holistic service understanding our Member’s needs.
The Outcome Star acts as a form of evaluation allowing Libertus to continually evaluate the service we provide and to identify areas in which we can improve.
